    How to Use this Form
    To start your report, select a Primary Concern Type. You will be able to include additional concerns in your report.   

    What Happens When I Submit a Report?
    Depending on the concerns listed, your report will either be sent to the Office of the Dean of Students or the Office of Student Persistence and an appropriate action will be taken within 3-5 business days. 

    Your report will not generate an automated e-mail to the student(s) of concern.
